ONCB · Orthopaedic Nurses Certification Board

Orthopaedic Nurse Certified (ONC / ONC-A)

ONC

The ONC is the Orthopaedic Nurses Certification Board's specialty credential for registered nurses working in orthopaedic and musculoskeletal care — from joint-replacement units and spine surgery floors to fracture clinics and sports-medicine settings. Many Magnet-recognized hospitals and orthopaedic service lines now treat ONC as a hiring preference or a clinical-ladder requirement, and the credential travels with you across employers. This page breaks down exactly what the ONC exam tests, how it's delivered through ProctorU, and how to get it done right the first time.

EXAM FORMAT

How the ONC exam is built — at a glance

150

Questions total

All multiple choice. 135 questions count toward your score; 15 are unscored pilot items seeded throughout the form, so you can't tell which is which.

97

Raw score to pass

You need at least 97 of the 135 scored questions correct — roughly 72%. The standard is criterion-referenced, not graded on a curve against other candidates.

3h

Time limit

Three hours for the full 150 items — about 72 seconds per question. There is no section-by-section timer; you manage the whole window yourself.

Two blueprints in one form

Every ONC item is classified two ways at once: by the musculoskeletal health condition it concerns (degenerative, trauma, sports, metabolic, inflammatory, pediatric, tumors, neuromuscular) and by the nursing task it tests (pain management, complications, self-care education, activity/positioning, nutrition, psychosocial support). The blueprint fixes how many questions fall into each bucket.

ONC vs ONC-A

ONC is the entry credential earned by passing this written exam. ONC-A (Advanced) is not an exam — it is awarded through a professional portfolio review for nurses who already hold ONC and practice at an advanced level. So when people say "the ONC exam," they mean this 150-question test; ONC-A is a later, portfolio-based step.

WHAT'S TESTED

Health-condition domains — bars show each area's approximate share of the 135 scored questions

Degenerative Disorders

38%

The largest domain (~51 questions): osteoarthritis, joint replacement, degenerative spine disease, and the post-operative care that dominates orthopaedic units.

Orthopaedic Trauma

19%

~26 questions on fractures, casting and traction, compartment syndrome, fat embolism, and acute trauma management.

Sports Injuries

11%

~15 questions on ligament and tendon injuries, ACL repair, sprains, strains, and return-to-activity care.

Metabolic Bone Disorders

9%

~12 questions on osteoporosis, osteomalacia, Paget's disease, and bone-density management.

Inflammatory Disorders

8%

~11 questions on rheumatoid arthritis, osteomyelitis, gout, and inflammatory joint conditions.

Congenital / Pediatric

5%

~7 questions on scoliosis, developmental hip dysplasia, clubfoot, and pediatric orthopaedic care.

Musculoskeletal Tumors

5%

~7 questions on benign and malignant bone tumors, limb-salvage surgery, and oncologic orthopaedic nursing.

Neuromuscular

4%

~6 questions on muscular dystrophy, spinal cord and peripheral nerve conditions affecting movement.

Layered over those conditions, the same items also test nursing tasks: pain management (35–45 questions) and complication prevention (31–41 questions) carry the most weight, followed by self-care education, activity and positioning, then smaller buckets for nutrition and psychosocial support. Percentages above are approximate, derived from the published question counts.

THE ONC CREDENTIAL FAMILY

How ONCB's orthopaedic nursing credentials fit together — and which ones involve a proctored exam

Credential Code How it's earned Exam help on this page?
Orthopaedic Nurse Certified ONC 150-question proctored written exam (this page) Yes
Orthopaedic Nurse Certified – Advanced ONC-A Professional portfolio review (no exam); requires existing ONC Portfolio support
Orthopaedic Nurse Practitioner – Certified ONP-C Separate APN-level proctored exam for nurse practitioners Ask us

Most RNs start with ONC. ONC-A is a later step that recognizes advanced practice through a reviewed portfolio rather than a sit-down test, so it cannot be "taken" the way the ONC exam can — our help for ONC-A focuses on portfolio preparation, while the proctored ONC exam is where our pay-after-you-pass model applies.

DELIVERY & PROCTORING

Two ways to sit the ONC — and what to expect on test day

At a Meazure Learning test center

ONCB offers year-round, computer-based testing through Meazure Learning's national test-center network. Staff verify your current photo ID, store your belongings, and monitor the room. Once your application is approved you have a 90-day eligibility window to schedule and sit the exam.

Online, via ProctorU Live Remote Proctoring

You can also sit the ONC from home through ProctorU (Meazure Learning), monitored live by a human proctor over webcam. You must install the Guardian secure browser, pass a connection and system check, present a current unexpired ID, and complete a room scan before the exam unlocks.

ID & identity check

A current, unexpired government photo ID matching your application name exactly. Since January 2023, ProctorU's Meazure Learning LRP no longer accepts expired IDs.

Environment scan

A clear desk, no notes, no phone within reach, and a webcam scan of the room via the Guardian browser. No one else may enter while you test.

During the exam

Stay in frame and on-camera the whole three hours. Looking off-screen, talking aloud, leaving the seat, or losing connection can flag the session for review.

WHO SHOULD TAKE THIS

ONC is built for experienced orthopaedic RNs

  • RNs on joint-replacement, spine, or orthopaedic surgical units
  • Fracture clinic, sports-medicine, and ortho rehab nurses
  • Nurses pursuing a clinical-ladder promotion or pay differential
  • Staff at Magnet hospitals where certification rates are tracked
  • Ortho OR, critical-care, and emergency nurses formalizing expertise

ELIGIBILITY & DIFFICULTY

There are hard prerequisites — and the bar is real

  • Current, full, unencumbered US RN license (or approved equivalent)
  • At least two full years of practice as an RN
  • 1,000+ hours of orthopaedic nursing practice in the past 3 years
  • Failing candidates must wait 90 days before re-testing

Difficulty: ONC is hard for a specific reason — bedside expertise does not automatically map to the exam. Many strong floor nurses know their own unit deeply but get caught out by domains they rarely touch (metabolic bone disease, tumors, pediatric ortho) and by the pharmacology and pathophysiology framing of pain and complication questions. Three hours of dense, application-style items after a full shift is its own challenge — which is exactly the pressure point our help is built to remove.

FREQUENTLY ASKED

Straight answers about the ONC orthopaedic nursing exam

What score do you need to pass the ONC exam? +
The ONC exam has 150 questions, but only 135 are scored — the other 15 are unscored pilot items. You need a raw score of at least 97 correct out of the 135 scored questions to pass. The ONCB uses a criterion-referenced passing standard, so you are measured against a fixed competency benchmark, not against other candidates.
Can I take the ONC exam online from home? +
Yes. The ONCB offers the ONC exam through Live Remote Proctoring delivered by ProctorU (Meazure Learning) as well as at Meazure Learning computer-based test centers. The online option requires the Guardian secure browser, a webcam check-in, a current unexpired photo ID, and a clear room scan before the exam unlocks.
What are the eligibility requirements for the ONC certification? +
You must hold a current, full, and unencumbered RN license in the United States or its possessions, have at least two full years of experience practicing as an RN, and have completed a minimum of 1,000 hours of orthopaedic nursing practice within the past three years. There is no separate degree requirement beyond active RN licensure.
What is the difference between ONC and ONC-A? +
ONC (Orthopaedic Nurse Certified) is earned by passing the 150-question ONCB written exam. ONC-A (Advanced) is not exam-based — it is awarded through a professional portfolio review for nurses who already hold ONC and want to demonstrate advanced practice. This page covers the ONC examination, which is the gateway credential for orthopaedic nurses.
How long is the ONC certification valid and can I retake the exam? +
ONC certification is valid for a five-year term, after which you recertify through contact hours and practice hours. If you do not pass, you may re-test after a 90-day waiting period. Once your application is approved you have a 90-day window to schedule and sit the exam.
Do I pay Exam Assist before or after I see my ONC result? +
You settle only after your verified passing result is confirmed. There is no upfront fee — you share your exam details, receive an honest feasibility answer, and decide before any money changes hands. We advertise a guaranteed pass with money back if you do not pass; we offer a transparent, results-first arrangement.
