CNOR · CNAMB · CFPN · CSSL
The CNOR is the gold-standard credential for operating-room nurses, awarded by the Competency & Credentialing Institute (CCI) and recognised by AORN-affiliated hospitals, Magnet-designated facilities, and surgical-services employers across the United States. Many OR units now require or financially reward CNOR for charge-nurse and circulator roles, and travel-nursing contracts often pay a premium for it. This page breaks down the CNOR exam and its sibling perioperative credentials — CNAMB, CFPN, and CSSL — and how to get certified the first time.

How the CNOR perioperative exam is built — at a glance
200
185 questions count toward your score; 15 are unidentified pretest items CCI uses to validate future content. You cannot tell which is which.
620
Correct answers convert to a scaled score from 200 to 800. You need at least 620, so there is no fixed percentage that guarantees a pass.
3h45
A single 3-hour-45-minute block at a PSI test center or via Remote Secure Proctored Exam — roughly 70 seconds per question.
To sit the CNOR you need a current, unrestricted RN license plus at least 2 years and 2,400 hours of perioperative experience, including a minimum of 1,200 hours in the intraoperative setting. Once CCI approves your application you must schedule and test within 90 days — a tight window many candidates underestimate while juggling OR shifts.
Unlike the NCLEX, the CNOR is a fixed-length, non-adaptive exam: everyone answers a full 200 questions drawn across seven content domains. Because difficulty varies slightly between forms, CCI uses scaled scoring so a 620 represents the same competency on every version of the test.
Seven CNOR content domains — bars show each domain's share of the scored questions
The largest domain: managing the surgical field, positioning, surgical counts, specimen handling, and circulating/scrub responsibilities during the procedure.
Sterile technique, instrument and supply processing, environmental controls, and the AORN guidelines that govern asepsis in the OR.
Patient assessment and nursing diagnosis before and after surgery — history, risk factors, and PACU handoff considerations.
Surgical time-outs, handoffs, the perioperative record, and team communication that keeps the patient safe across the surgical journey.
Recognising and responding to intraoperative crises — malignant hyperthermia, hemorrhage, cardiac arrest, fire, and rapid conversions.
Developing an individualized perioperative plan of care and identifying the expected outcomes that frame nursing interventions.
A seventh domain, Professional Accountability (about 6%), covers ethics, advocacy, and quality improvement. Because Intraoperative Activities alone is roughly a third of the scored questions, candidates who circulate or scrub daily have a real edge — but the exam also rewards textbook AORN guideline knowledge that day-to-day practice can leave rusty.
CNOR is the flagship — but CCI runs a full ladder of perioperative credentials. We help with all four.
| Credential | Code | Who it's for | Questions |
|---|---|---|---|
| Certified Perioperative Nurse | CNOR | Experienced OR / circulating & scrub nurses | 200 (185 scored) |
| Certified Ambulatory Surgery Nurse | CNAMB | Nurses in ambulatory / same-day surgery settings | 200 (185 scored) |
| Certified Foundational Perioperative Nurse | CFPN | Early-career nurses, under 23 months in the OR | 120 (100 scored) |
| Certified Surgical Services Leader | CSSL | Perioperative managers, directors & service-line leaders | Multiple-choice |
All four are administered by CCI and delivered through the same PSI test-center and Remote Secure Proctored Exam channels. The CFPN is a natural stepping stone toward CNOR for newer nurses, while CSSL is the only perioperative management credential in the family. Tell us which one your role calls for and we'll map the prep accordingly.
Two ways to sit the CNOR — and what to expect on test day
You sit the CNOR in a quiet, monitored room at a PSI testing center, available Monday through Saturday year-round (excluding holidays). Staff verify your government-issued ID, store your belongings, and supervise the room while you work through the 200 questions on a center workstation.
You take the CNOR from a private room at home through PSI's secure browser (PSI Bridge), monitored by a live remote proctor. You'll run a system check, an ID verification, and a webcam room scan before the exam unlocks. Stable internet and a quiet, single-occupant room are mandatory.
A valid, unexpired government photo ID whose name matches your CCI application exactly. The proctor captures a photo before the secure browser launches the exam.
A clear desk, no second monitor, no phone within reach, and a full webcam scan of the room. No notes, scrubs reference cards, or other people are permitted during the session.
Stay in frame and on-camera throughout the 3h45m block. The PSI Bridge browser locks down your machine; leaving the seat, talking aloud, or losing connection can flag or void the attempt.
The CNOR validates the operating-room nurse role
Hard prerequisites, and a genuinely demanding exam
Difficulty: CNOR tests recall of AORN guidelines, evidence-based practice, and rationale most nurses apply intuitively but rarely articulate from memory. The gap between "I do this every shift" and "I can answer 185 scaled questions on it in under four hours" is where strong clinicians stumble — exactly the pressure point our help is built to remove.
The CNOR sits between a busy OR nurse and a certification-pay bump, a charge role, or a Magnet requirement — and the 90-day testing window leaves little room to fail. Exam Assist pairs you with a vetted perioperative specialist and works on a pay-after-you-pass model, so the risk sits with us, not you. No upfront fee, guaranteed results: Exam Assist handles the sitting end to end, and you settle only after the verified result.
Tell us which credential you're after (CNOR, CNAMB, CFPN, or CSSL), your delivery method, and your test date. Takes a couple of minutes over WhatsApp, Telegram, or Discord.
We review your timeline and the 90-day window and tell you plainly whether it's realistic — before any money is discussed. If it isn't a fit, we say so.
Exam Assist handles the sitting end to end. You're matched with a perioperative specialist who maps the work around the seven CNOR domains, AORN guidelines, and the PSI Bridge environment — discreetly and confidentially.
