CCRN Critical Care Nursing

CCRN · Adult / Pediatric / Neonatal

The CCRN is the American Association of Critical-Care Nurses' flagship credential for bedside nurses who care for acutely and critically ill patients. Earning the three letters after your name signals validated expertise to charge nurses, hiring managers, and Magnet-status employers — and at many hospitals it unlocks clinical-ladder pay tiers, ICU float eligibility, and differential bonuses. It is also one of the few specialty exams that quietly weeds out strong nurses on the day, because the questions force fast clinical judgment rather than recall. This page breaks down exactly what the CCRN tests, how AACN delivers it through PSI, and how to get it done right.

EXAM FORMAT

How the CCRN is built — at a glance

150

Total questions

All four-option multiple choice. 125 count toward your score; 25 are unscored pretest items seeded throughout the exam, and you can't tell which is which.

3h

Seat time

A single 3-hour testing window — roughly 72 seconds per question. There is no separate break built into the clock, so pacing is part of the challenge.

3

Patient populations

Adult, Pediatric, and Neonatal versions share the same blueprint and length — only the clinical scenarios change to match your population.

Format & question style

The CCRN is linear (not adaptive): every candidate answers the same fixed number of items, and you can flag and revisit questions within the 3 hours. Items are scenario-driven — a short patient vignette with vitals, labs, or a rhythm strip — that ask you to prioritize an intervention rather than define a term.

How scoring works

Only the 125 scored items count. AACN sets the passing standard psychometrically with PSI, so the Adult cut is roughly 83 correct out of 125 but can shift slightly by form. You receive an immediate preliminary pass/fail result at the testing station, with a domain-level breakdown of your performance — there is no percentile or letter grade.

WHAT'S TESTED

Two top-level domains — bars show each section's share of the scored items

1

Clinical Judgment

80%

The body-system clinical content: cardiovascular (the heaviest single area), pulmonary, neurology, gastrointestinal, renal/genitourinary, endocrine, hematology/immunology, integumentary, musculoskeletal, behavioral/psychosocial, and multisystem (sepsis, shock, multi-organ failure). You assess, prioritize, and intervene under time pressure.

2

Professional Caring & Ethical Practice

20%

Built on AACN's Synergy Model: advocacy and moral agency, caring practices, collaboration, systems thinking, response to diversity, clinical inquiry, and facilitation of learning. Easy to under-study because it isn't "bedside science" — and exactly where many strong clinical nurses lose points.

Clinical Judgment body systems (Adult blueprint)

Approximate weights of the scored items — heaviest areas first. Pediatric and Neonatal blueprints mirror this structure with population-specific emphasis.

Body system Approx. weight
Cardiovascular~17%
Pulmonary~15%
Multisystem (sepsis, shock, MODS)~14%
Neurology, GI, Renal/GU, Endocrine, Heme/Immune, Integumentary, Musculoskeletal, Behavioral/Psychosocialremaining ~34%

Cardiovascular and pulmonary together are roughly a third of your scored items, so weak ECG interpretation, hemodynamics, or ventilator management quickly caps a score. The trap is the 20% Professional Caring section: the answers turn on the Synergy Model's language, not clinical instinct, and candidates who skip it routinely fall a few points short.

DELIVERY & PROCTORING

Two ways to sit the exam through PSI — and what to expect on test day

At a PSI test center

After AACN approves your application you schedule through PSI and sit the exam in a quiet, monitored room at one of hundreds of US testing centers, open Monday through Saturday. Staff verify your government photo ID, store your belongings, and provide a workstation and on-screen tools — no notes or devices allowed.

Online via PSI Live Remote Proctoring

You can take the CCRN from a private room at home through PSI's Live Remote Proctoring (LRP). It runs in PSI's secure browser, monitored by a live proctor over your webcam and microphone. You'll complete a system check, ID verification, and a room scan before the exam unlocks — and a stable connection is essential, since drops can halt the session.

ID & identity check

A valid, unexpired government photo ID with a name that exactly matches your AACN application. The proctor captures your photo and ID before the exam begins.

Environment scan

A clear desk, no second monitor, no phone or notes within reach, and a full webcam scan of the room. No one else may enter while you test, and the secure browser locks down your machine.

During the exam

Stay in frame and on-camera throughout. Leaving your seat, talking aloud, looking off-screen, or losing connection can flag or void the session under PSI's rules.

WHO SHOULD TAKE THIS

The CCRN is built for direct-care critical care nurses

  • ICU, CVICU, MICU/SICU, and ER nurses caring for critically ill patients
  • PICU and NICU nurses pursuing the Pediatric or Neonatal credential
  • Nurses at Magnet or Pathway-to-Excellence hospitals that reward certification
  • Nurses climbing a clinical ladder or chasing a pay differential and bonus
  • Travel nurses who need a portable specialty credential for ICU contracts

ELIGIBILITY & DIFFICULTY

A license plus verified bedside hours — and a tough bar

  • Current, unencumbered US RN or APRN license
  • 1,750 direct-care hours in the past 2 years (875 in the most recent year)
  • Or 2,000 hours in the past 5 years (144 in the most recent year)
  • Hours must match the population — Adult, Pediatric, or Neonatal

Difficulty: The CCRN rewards judgment, not memorization. Bedside experience helps, but the exam tests rapid prioritization across every body system in three hours, with cardiovascular and pulmonary content dense in ECGs, hemodynamics, and ventilator math — plus a 20% Synergy-Model section that surprises strong clinicians. That mismatch between competent nursing and exam-day performance is exactly the pressure point our help is built to remove.

FREQUENTLY ASKED

Straight answers about the CCRN exam

What is the passing score for the CCRN exam? +
The CCRN is scored on its 125 scored questions (150 total, with 25 unscored pretest items). For the Adult exam the cut score is roughly 83 of 125 correct, though AACN sets the exact passing standard psychometrically for each form, so it can vary slightly. You receive a scaled pass or fail result, not a percentile rank.
What is the difference between Adult, Pediatric, and Neonatal CCRN? +
AACN offers the CCRN in three patient populations — Adult, Pediatric, and Neonatal. The exam structure, length, and eligibility hours are identical; only the clinical content reflects the population you care for. You apply for the one that matches your bedside practice, and each is a separate credential.
Can I take the CCRN exam online from home? +
Yes. AACN delivers the CCRN through PSI, and you can sit it either at a PSI test center or from home via PSI Live Remote Proctoring. The at-home option requires a computer with a webcam, microphone, and stable internet, a government photo ID check, and a 360-degree scan of a private, distraction-free room before the exam unlocks.
What are the eligibility requirements for the CCRN? +
You need a current, unencumbered US RN or APRN license plus verified direct-care hours with acutely or critically ill patients: either 1,750 hours in the previous 2 years (with 875 in the most recent year) or 2,000 hours in the previous 5 years (with 144 in the most recent year). The hours must match the population — Adult, Pediatric, or Neonatal — you are testing in.
How long is CCRN certification valid and how do I renew? +
CCRN certification is valid for 3 years. You renew either by earning Continuing Education Recognition Points through the Renewal by Synergy CERPs program or by retaking the CCRN exam, while maintaining your RN license and clinical hours over the renewal period.
Do I pay Exam Assist before or after I see my CCRN result? +
You settle only after your verified passing result is confirmed. There is no upfront fee — you share your exam details, receive an honest feasibility answer, and decide before any money changes hands. We advertise a guaranteed pass with money back if you do not pass; we offer a transparent, results-first arrangement.
