Scores are reported on a 200–800 scale and you pass at 500. The cut is set by the Angoff method, so it is a fixed competency bar rather than a simple percentage of questions correct.
You have 150 minutes for the full question set, all delivered linearly (not adaptive) — you can flag and revisit items within your time. The clock is the real constraint: many candidates report the application-style scenarios eat time, so pacing of roughly a minute per item with margin for review is essential.
Only the scored items count toward your scaled score; pretest items are excluded after psychometric review. There is no separate pass mark per domain — the single 200–800 scaled score determines pass or fail, and a preliminary result is shown at the test center or online session before IHI confirms it officially.

IHI does not publish a fixed percentage weight for each domain; the bars above reflect the relative emphasis candidates report, with Systems Thinking and Human Factors carrying the heaviest application-style load. Crucially, the CPPS is not a recall test — most items are scenario-based, asking you to choose the best next action in a realistic safety situation, which is where unprepared candidates lose the most points.
Two ways to sit the CPPS — and what to expect on test day
After IHI approves your application, you schedule a seat at a PSI Assessment Center in the US or internationally. Staff verify your government-issued ID, store your belongings, and monitor the room. This is the most predictable option if your home setup or employer's network is uncertain.
You take the exam from a private room at home through the PSI Secure Browser, monitored by a live remote proctor. You run a system-compatibility check, a webcam room scan, and an ID check first. IHI specifically warns against using an employer-issued laptop, because IT security policies often block the Secure Browser from launching.
PSI's compatibility test must pass before exam day: supported OS, working webcam and mic, and a stable connection. Use a personal device, not a locked-down work computer.
