CCB · Compliance Certification Board (HCCA / SCCE)

Certified in Healthcare Compliance

CHC · CHPC · CHRC

The CHC, awarded by the Compliance Certification Board (CCB) and administered through the Health Care Compliance Association (HCCA), is the benchmark credential for healthcare compliance officers across hospitals, health systems, payers, and physician groups. Many compliance, privacy, and audit roles list CHC as a requirement or strong preference, and it sits alongside two siblings — CHPC for privacy and CHRC for research compliance. Unlike a one-and-done test, it gates a regulated career and must be renewed every two years. This page breaks down what each exam covers, how PSI delivers it, and how to get it done right the first time.

EXAM FORMAT

How the CHC examination is built — at a glance

120

Questions on the form

Every CHC form holds 120 multiple-choice questions. Only 100 are operational; the other 20 are unscored pretest items that look identical, so you must answer all of them seriously.

75

Scaled pass mark

CCB sets the cut at a scaled score of 75 out of 100. There is no curve to a peer group — you clear a fixed bar of demonstrated competence across every content domain.

2h

Time limit

You have two uninterrupted hours — roughly one minute per question. The pacing is unforgiving when items are scenario-based and ask you to apply law rather than recall it.

What the questions feel like

CHC items are application-heavy: a short fact pattern about a hospital, billing arrangement, or audit finding, followed by "what should the compliance officer do." Many distractors are plausible-but-wrong, so rote memorization of the False Claims Act or Stark Law is not enough — you have to reason like a practitioner under the federal Sentencing Guidelines framework.

How scoring & results work

Your raw count of correct operational answers is converted to a scaled score out of 100, and 75 is the pass line. You get a preliminary pass/fail at the test center or end of the online session, with the official result and CHC designation confirmed by CCB after review. Fail and you can re-sit within your eligibility window for a reduced $75 re-exam fee.

THE CCB HEALTHCARE FAMILY

CHC has two siblings — same format, same renewal cycle, different focus

Credential Code Who it's for
Certified in Healthcare Compliance CHC The broad, general healthcare-compliance credential — the default for hospital and health-system compliance officers, auditors, and coordinators.
Certified in Healthcare Privacy Compliance CHPC Privacy specialists working on HIPAA, the protection of PHI, breach response, and patient-data governance.
Certified in Healthcare Research Compliance CHRC Clinical-research compliance professionals handling human-subject protections, IRB oversight, grants, and research billing.
Certified in Healthcare Compliance — Fellow CHC-F Experienced certified professionals who pursue CCB fellowship recognition through additional experience and continuing education — not a separate exam.

All three exams (CHC, CHPC, CHRC) share the 120-item, two-hour, scaled-75 structure, the same 20-CEU eligibility model, and the same two-year, 40-CEU renewal cycle. Pick the credential that matches the work you actually do — recruiters increasingly distinguish a privacy-focused CHPC from a general CHC.

WHAT'S TESTED

CHC content domains — bars show the approximate share each carries on the form

1

Compliance Program Administration

~25%

Building and running the seven elements of an effective compliance program: code of conduct, oversight, training, the compliance committee, and program effectiveness reviews under the OIG guidance.

2

Standards, Policies & Procedures

~20%

Drafting, communicating, and maintaining written standards; documentation, record retention, and translating federal regulation into workable policy across a health system.

3

Monitoring, Auditing & Risk

~25%

Conducting risk assessments, internal audits, and ongoing monitoring; coding and billing integrity; identifying overpayments and the 60-day refund rule.

4

Response, Investigation & Enforcement

~30%

Investigating reports, corrective action, discipline, and self-disclosure; the legal backbone — Stark Law, Anti-Kickback Statute, False Claims Act, HIPAA, EMTALA, and the federal Sentencing Guidelines.

Percentages are approximate and based on CCB's published CHC Detailed Content Outline; verify the exact blueprint and any updates on the HCCA site before you sit. CHPC re-weights toward HIPAA and privacy, and CHRC toward research and human-subject protections, but all draw on the same compliance-program backbone.

DELIVERY & PROCTORING

Three ways to sit the CHC — and what PSI expects on test day

PSI test center or HCCA/SCCE event

You can take the computer-based CHC at a local PSI testing center, or sit a paper/CBT exam at select HCCA or SCCE conferences and academies. Center staff verify your government ID, store your belongings, and monitor the room. This route suits candidates who want a controlled, distraction-free setting.

Online, proctored 24/7 via PSI

CCB's remote option lets you sit the CHC from a private room at home, available 24/7 through PSI Services. You install and launch the PSI Secure Browser, which locks down your machine, then a live proctor verifies your ID, runs a webcam room scan, and watches the session. No second monitor, no notes, no one else in the room.

System & secure browser check

Before exam day, run PSI's compatibility check and install the PSI Secure Browser. A weak webcam, blocked ports, or VPN can stop you from launching, so test your setup early.

ID & environment scan

A valid government photo ID matching your CCB application, plus a 360° room scan and a clear desk. Phones, smartwatches, and second screens must be out of reach.

During the exam

Stay on camera and in frame the entire two hours. No breaks are guaranteed, no talking aloud, and no leaving the seat — losing connection or breaking eye contact can flag the session for review.

WHO SHOULD TAKE THIS

CHC is built for healthcare compliance professionals

  • Hospital and health-system compliance officers and directors
  • Compliance auditors, analysts, and coordinators in healthcare
  • Privacy officers, HIM, and HIPAA leads (often pairing CHPC)
  • Attorneys, nurses, and coders moving into compliance roles
  • Research-compliance staff at academic medical centers (CHRC)

ELIGIBILITY & DIFFICULTY

There are real prerequisites — and the bar is high

  • 20 CCB-approved CEUs (≥10 live) earned in the prior 12 months
  • Qualifying compliance work experience (or a CCB-accredited program)
  • A CCB-approved application before you can schedule the exam
  • Working command of US healthcare law and regulation

Difficulty: CHC is hard because it spans a wide body of federal law — Stark, Anti-Kickback, False Claims, HIPAA, EMTALA — and asks you to apply it to messy scenarios, not just recall definitions. Many candidates know their own department well but get caught by domains outside their day job, all while pacing one minute per question under PSI lockdown. That breadth-under-pressure is exactly where our help is built to remove the risk.

FREQUENTLY ASKED

Straight answers about the CHC, CHPC, and CHRC

What score do I need to pass the CHC exam? +
The CHC exam is scored on 100 operational multiple-choice questions, and you need a scaled score of at least 75 out of 100 to pass. Your form also includes 20 unscored pretest questions that look identical to scored items, so you cannot tell which questions count — you have to treat all 120 as if they matter.
What is the difference between CHC, CHPC, and CHRC? +
All three are CCB credentials administered through HCCA. CHC (Certified in Healthcare Compliance) is the broad general healthcare-compliance credential. CHPC (Certified in Healthcare Privacy Compliance) focuses on HIPAA, privacy, and the protection of health information. CHRC (Certified in Healthcare Research Compliance) focuses on clinical research, human-subject protections, and grants compliance. They share the same exam format, eligibility model, and two-year renewal cycle.
Can I take the CHC exam online from home? +
Yes. CCB partners with PSI Services to offer remote online proctoring 24/7 for CHC, CHPC, and its other basic certifications. You take the exam from a private room on your own computer using the PSI Secure Browser, with a webcam room scan, a government-ID check, and a live proctor. You can also sit a computer-based test at a PSI test center or at select HCCA/SCCE conferences.
What do I need before I can sit the CHC exam? +
Before applying you must submit 20 CCB-approved continuing education units (CEUs) earned within the prior 12 months, with at least 10 of them from live training, and meet the compliance work-experience requirement (or complete a CCB-accredited university program). CCB reviews and approves your application before you are cleared to schedule the exam.
How long is the CHC certification valid and how do I renew? +
CHC certification is valid for two years. To renew you must earn and report 40 CCB CEUs within each two-year cycle, including a minimum of 20 from live events, then submit the renewal application and fee. Let it lapse and you generally have to re-sit the exam, so the credential is an ongoing commitment, not a one-time test.
Do I pay Exam Assist before or after I see my CHC result? +
You settle only after your verified passing score is confirmed. There is no upfront fee — you share your exam details, receive an honest feasibility answer, and decide before any money changes hands. We advertise a guaranteed pass with money back if you do not pass; we offer a transparent, results-first arrangement.
