The CCM is the most widely held case-management credential in the United States, issued by the Commission for Case Manager Certification (now branded The Commission). Hospitals, managed-care organizations, workers' compensation carriers, and insurance payers list it on RN and social-work case manager postings — and for many, the letters after your name are the difference between staying in direct care and moving into a higher-paid coordination role. Once The Commission approves your application, you book through Pearson VUE and pick either a physical test center or OnVUE online proctoring from home. The exam is offered in standing testing windows several times a year; you choose a date inside your approved window.
Your raw score on the 150 scored items is converted to a scaled score so that easier and harder forms are equated fairly. There is no published percentage cut and no domain breakdown for candidates who pass — you receive a straight pass or fail result. As a rule of thumb, candidates aim for around 70 percent correct.

The largest domain: levels and settings of care, transitions of care, utilization review, alternative care facilities, the case-management process, and coordinating services across the continuum of care.
Split from the old Domain 1 in the August 2025 blueprint: insurance and benefit programs, payer systems, and reimbursement models including Medicare, Medicaid, and managed care.
Psychological and social influences on a client's condition: behavioral health, support systems, family dynamics, cultural and spiritual factors, health literacy, and social determinants that shape adherence and outcomes.
Standards of practice, confidentiality and HIPAA, healthcare legislation, ethical decision-making, risk management, and the documentation and record-keeping rules that govern professional case management.
Outcome measurement, quality indicators, cost-benefit and case-load analysis, accreditation standards, and the data and metrics used to demonstrate the value of case-management interventions.
The smallest domain: vocational and physical rehabilitation principles, functional capacity, disability and return-to-work concepts, assistive technology, and coordinating rehabilitation services across the continuum of care.
Domain weights follow the current CCMC exam blueprint (updated August 2025, when the former Care Delivery domain was split into Care Management and Reimbursement Methods) and are approximate; The Commission periodically updates the content outline from its practice analysis. Because Care Management, Reimbursement, and Psychosocial together make up just over half the exam, a candidate strong only in clinical knowledge can still fail on reimbursement systems and social-determinant scenarios.

Two ways to sit the exam — and what OnVUE expects on test day
You sit the CCM in a quiet, monitored room at a Pearson VUE center. Staff verify your government-issued ID, store your belongings in a locker, and watch the floor by camera. An erasable noteboard is provided, and you take the exam on a center workstation under standard test-center rules.
OnVUE lets you take the CCM from a private room at home, monitored by a live proctor through the OnVUE secure browser. You run a system test, install the software, and complete check-in by photographing your ID and your testing space. No scratch paper or physical notes are allowed — an on-screen note tool is used instead.
A valid, unexpired government photo ID with a name matching your registration. You photograph your ID and four views of your room, and a proctor reviews them before the exam unlocks.
A clear desk, no second monitor, no phone or notes within reach, no headphones, and no one else in the room. OnVUE requires a single screen and disables other applications during the session.
Stay on-camera and in frame the entire 3 hours. Leaving the seat, talking aloud, reading questions out loud, or losing connection can pause or flag the session — there are no scheduled breaks built into OnVUE.
