What is the ACSM CPT exam?
The ACSM Certified Personal Trainer (ACSM-CPT) exam is a 135-question, 150-minute multiple-choice exam that earns the personal-training credential from the American College of Sports Medicine - the credential ACSM describes as the most respected in the health and fitness industry. It costs US$410 for non-members (US$310 for members, US$205 per retest), passes at a scaled 550 on the 200-800 scale, and can be taken online from home via Pearson OnVUE or at a Pearson VUE test center.
The official FAQ describes the format plainly: the examination for each level of certification is composed of multiple-choice questions drawn from the Job Task Analysis found in ACSM's Guidelines for Exercise Testing and Prescription, 12th Edition. Fifteen of the CPT's 135 items are unscored pretest questions.
Delivery is stated word-for-word on ACSM's CPT page: the ACSM Certification exam can be taken in-person at a Pearson testing center, or take it online from the comfort of your own home, office or workplace.
How is the ACSM CPT exam structured?
The Handbook's Appendix C publishes the exact shape: a 150-minute seat time with 135 items, of which 120 are scored and 15 are non-scored. That is 67 seconds per item across the sitting - a pace that punishes second-guessing more than difficulty.
All items are multiple-choice. A standard or scientific calculator is provided within the exam interface, and metabolic equations and other applicable formulas are supplied in the exam itself (from the Guidelines' Appendices C and D), so memorizing equations is not the task - applying them under time pressure is.
On the OnVUE route there are no breaks: the Handbook states that candidates who leave their computer for any reason have the exam terminated by the proctor and forfeit the registration. Water in a clear glass is the only permitted desk item; the digital whiteboard replaces scratch paper.
What domains does the ACSM CPT exam cover?
Four performance domains, weighted in the Handbook's 2024 outline: Exercise Programming and Implementation 43%, Initial Client Consultation and Assessment 25%, Exercise Leadership and Client Education 22%, Legal and Professional Responsibilities 10%.
Read the weights as a study plan. Nearly half the scored exam is programming - FITT-based cardiorespiratory and resistance prescription, progressions, regressions, and modifications. A quarter is screening and assessment: pre-participation health screening, risk stratification, fitness testing protocols, and interpretation. Who is eligible to take the ACSM CPT exam?
Three requirements, all published: 18 years or older, a high school diploma or equivalent, and a current adult CPR/AED certification that includes a live, in-person practical skills examination component - American Heart Association or American Red Cross format. Beginning in 2027, all ACSM certifications additionally require first aid training.
The application route runs through the MyACSM portal: submit the application and documents, wait for approval, and a three-month eligibility window starts from approval or full payment, whichever is later. Extensions cost US$50 per three months, up to three (a maximum of one year).
What does the ACSM CPT exam cost?
The published Exam Only fees, effective January 1, 2025: US$410 for non-member candidates, US$310 for ACSM members, US$205 for a retest - every fee includes a US$50 non-refundable application fee. Membership pricing requires an active ACSM membership at application time.
The retake economics matter more than the sticker: a 15-day wait between attempts, at most 4 attempts in any 12-month period, a 12-month lockout after a fourth failure, and the full retest fee each time. A two-attempt pass totals US$615 non-member; three attempts total US$820.

Can you take the ACSM CPT exam online from home?
Yes. Pearson VUE runs a dedicated ACSM OnVUE page headed Online testing for American College of Sports Medicine (ACSM), and ACSM's own CPT page offers the online route in the same sentence as the test-center route.
The published minimums: Windows 10 or macOS 14 or higher; working webcam, microphone, and speaker with no headphones or headsets; one display screen only; at least 6 Mbps download and 2 Mbps upload; and the ability to close all applications except OnVUE. Check-in takes two IDs - a primary with photo and signature, a secondary with a signature - and roughly 15 minutes after a completed system test.
OnVUE testing is not currently offered in China, Cuba, India, Iran, Japan, Korea, or Sudan. ACSM recommends a personal computer on wired ethernet - work computers and unreliable wireless are the two most frequent causes of technical failures.
How hard is the ACSM CPT exam?
ACSM publishes no pass rates, so honest difficulty talk is structural: 120 scored items across four domains with 43% of the weight in programming, a 550 scaled passing point on a 200-800 scale, and a 67-second-per-item pace with 15 unscored items silently mixed in.
The failure patterns are consistent: candidates who study evenly instead of weighting programming, candidates who meet the metabolic equations for the first time mid-exam, and candidates undone by logistics - an OnVUE minimum missed at check-in means the exam cannot be taken and the fee is forfeited.
